If Davids parents were doctors, why do you think they ran away?
I think they ran away so they could reverse the operation for other people. So that anyone could have the choice to become pretty or not. Then they made the Smoke for people who didnt want to have the operation could stay without being caught and forced to be changed into somthing they dont want to be changed into.
